We gladly accept assignment of benefits from all PPO plans and will file claims on your behalf. 

Here is our insurance participation information. 

All our providers are in-network with the following major insurance carriers: 

1. Delta Dental (Premier status) 

2. Humana PPO, HMO Medicare 

3. Cigna PPO and Medicare 

As a token of our appreciation for the continued support from the community, we also accept the State of Illinois Medicaid plans for both adults and children, providing a variety of payment methods.

We gladly accept various payment methods, including Cash and Credit Card payments. If you require payment plans, please consult our front desk to make the necessary arrangements. We also encourage our patients to utilize assignment of benefits for insurance participation, particularly with PPO plans. We expect all our patients to settle the payment for the amounts they are responsible for at the time of service. This helps us avoid sending monthly statements or making constant calls for collection, allowing us to dedicate more time to patient care!
