We do accept assignment of benefits from all PPO plans and file claims on your behalf.  

Here is our insurance participation information.

All our providers are in network with the following major insurnace carriers.

1. Delta Dental (Premier status)

2. Humana PPO, HMO Medicare

3. Cigna PPO and Medicare

Also, as our way of appreciating the continued support from the community, we do accept the State of Illinois Medicaid (both adults and children) plans.

We gladly accept Cash and Credit Card payment.  

If you need payment plans, please consult our front desk to make the arrangement.  

 We expect all our patients to make the payment for the amount they are responsible for at the time of service.  This way, we don't have to send you monthly statement or constantly call you to collect.  We can use that time to focus more on our patient care!
